Documents required by the industrial entrepreneurs while submitting application for grant of sales Tax incentive.

1.  Application form duly filled in.

2.  Copy of supporting documents in respect of commencement of first investment and details thereof (in case of land, sale/lease deed be submitted alongwith the proof of payment duly countersigned by G.M./P.M.

3.  Copy of agreement executed with GRIDCO along with the certificate on supply of power along with bills of GRIDCO regarding consumption of electricity.

4.  Copy of PMT/letter of intent/Industrial Licence/IBM No. if any.

5.  Copy of the project Report.

6.  Copy of appraisal report of Financing Institutions.

7.  Copy of Status of Company (Memorandum and Articles of Association).

8.  List of Board of Directors in case of medium/large scale industries.

9.  OST and CST registration certificate to be submitted.

10.Date of Ist. Purchase of raw materials (items and value) (copy of bills be submitted duly countersigned by G.M/P.M.

11.NOC from the Financing Institutions.

12.In case of Large/Medium Industry photocopy of production certificate to be submitted.

13.In case of priority industry/Poineer Industry and Labour industry (Photo copy of the Certificate issued by D.I., Orissa to be submitted.

14.In case of tourism related activity photo copy of the letter of Department of Tourism to be submitted along with the application form.

15.Month and item-wise statement on purchase, production, sale and stock with quantity and value.

16.Month wise statement of electricity consumption of the unit.

17.Statement alongwith copies of the bills and copy  of aunthenticated money receipts of machineries and other fixed assets.
